Telephonic Nurse Case Manager (RN) - Atlanta, GA 30319

We are currently hiring for evening shifts (Noon-9 or 1-10) Monday through Friday plus Saturday or Sunday daytime rotations. We also offer the ability to work 4-10 hour shifts, which also include Saturday/Sunday rotations. All positions are full-time onsite in our Atlanta office on Ashford-Dunwoody Road, across from the Perimeter Mall. Primary responsibilities include: - Making outbound calls to assess members' current health status
- Identifying gaps or barriers in treatment plans
- Providing patient education to assist with self management
- Interacting with Medical Directors on challenging cases
- Coordinating care for members
- Making referrals to outside sources
- Coordinating services as needed (home health, DME, etc)
- Educating members on disease processes
- Encouraging members to make healthy lifestyle changes
- Documenting and tracking findings
- Making post-discharge calls to ensure that discharged member receive the necessary services and resources

Requirements include: - A current, unrestricted RN license in the State of Georgia (or the ability to obtain through reciprocity)
- 3+ years experience in a hospital setting, acute care, direct care experience OR experience as a telephonic Case Manager for another insurance company; at least one year of clinical experience must be dealing with adult patients
- Computer proficiency, to include the ability to type while speaking on the phone
Additional assets include: - BSN
- Certified Case Manager (CCM)
- A background in managed care
- Case or disease management experience
- Experience/exposure with discharge planning
- Bilingual (Spanish/English) skills
- Medical/Surgical, Home Health, Diabetes or Cardiac experience is strongly preferred (certification in one of those specialties is a plus)
- Strong organizational skills and multitasking abilities will be keys to success.
