CareMore's Primary Care & Collaboration division oversees strategy, operations, and care delivery in our primary care markets (Iowa, Tennessee, Connecticut), where CareMore builds and runs capitated primary care medical groups and currently serves over 22,000 high-risk Medicaid and Medicare patients, as well as our collaboration with Emory Health System, which brings the CareMore model to over 20,000 Medicare Advantage patients in Georgia. The division also supports the development of new markets and models in the Primary Care & Collaboration area, including multiple new markets planned in 2018 and 2019. Our comprehensive approach to care includes extensivists managing acute and post-acute episodes of care, primary care clinicians, behavioral health clinicians, care management & engagement with case managers and community health workers, and mobile home-based care. The Therapist (LCSW), CareMore Primary Care will serve as a behavioral health specialist for a panel of complex Medicaid Patients. This individual will an integral member of the behavioral health team and will provide real-time clinical guidance and support to the primary care team in identifying, assessing, and managing patients with mild to moderate BH issues, and appropriately referring patients with severe BH issues.

  • Assess, diagnose, and treat individuals and families with behavioral health conditions, ranging from serious mental illness to substance abuse, or the potential for such conditions, using their full scope of therapeutic skills
  • A true patient-centered approach, from a bedside manner that puts patients, caregivers, and team members at ease
  • A collaborative team member and effective communicator
  • Willingness to operate in an ambiguous environment, without continuous supervision
  • Experience operating in an integrated behavioral health delivery model
Essential Duties and Responsibilities
  • Performs mental status examinations, biopsychosocial assessments, and psychotherapyReviews psychological diagnostic tests to identify and assess patient's clinical problems and health care needs
  • Records assessment findings and develops treatment plan including preventive health measures Discuss case with the medical and behavioral health physician leadership and other care team members (social work, therapists, care managers) to prepare and enact a comprehensive patient care plan
  • Submits the health care plan and goals of individual patients for periodic review and evaluation by the physician leadership
  • Refers patients to the Physician for consultation or specialized health resources for treatment
  • Maintain complete medical records, including history, physical exams, and telephone message summaries timely and accurately
  • Attend behavioral health and/or medical rounds at the care center, perform e-consults as needed, collaborate with care center teams on both medical and behavioral health aspects of care

Work Location & Travel Expectations

Based in local market, with local travel.




Qualifications
    • MS or MA in social work, counseling, or a related behavioral health field or a degree in nursing or PhD in Psychology
    • 3 years' experience with facility-based and/or outpatient psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience, which would provide an equivalent background
    • Current unrestricted license as an LMHC, LCSW, LPC (as allowed by applicable state laws), LMFT, LMSW or Psychologist required.
