Eliot's Emergency Service Program is seeking an experienced, talented, clinical coordinator.
Provide clinical coordination for designated DMH funded programs and identified frequent utilizers of Emergency Service.
Collaborate with Eliot staff, other mental health organizations, substance abuse providers, hospitals, police, outpatient clinicians and other community providers to enhance community tenure and stability of individuals served.
Develop service and support plans that support each individual's diversion from acute care and integration into community based services.
Provide short term solution focused clinical intervention and treatment for individuals and families as indicated for crisis management.
Act as liason for DMH funded programs, meeting with staff and clients regualary to develop and implement Safety and Risk plans.
Participate in Administrative On-call rotation for Emergency Services Program.
|Schedule: Monday-Friday 9am-5pm|
Requires a Masters’ Degree in a related field and independent clinical license.
Must have at least four (4) years of experience working with individuals with mental illness and co-occurring substance abuse issues, including two (2) years in a supervisory position. Strong understanding of DMH programs, practices and individuals served. Ability to connect clients to community resources and provide short term treatment, referral and connections to sustained supports. Proven ability to develop community connections with other providers, stakeholders, police departments and family members. Ability to develop individualized interventions and plans for individuals with emotional, mental health and/or substance abuse issues who are in crisis and/or involved with the criminal justice system.
