Under general to limited supervision, perform social services functions at an advanced level to people applying for services, their families and the agencies that support them
The Intake Coordinator conducts screenings for participants new to the waivers and provides support to the other Social Workers in Intake & Evaluation.
The position performs comprehensive social work and psychosocial assessment of consumers based on interviews and data collected through history, observation, or psychosocial examination, and analysis of diagnostic data.
The Intake Coordinator provides educational/instruction/technical assistance to parents, consumers, and providers assistance on various social services issues (e.g., available services, benefits, ect.).
The Intake Coordinator communicates with agency personnel, service providers, support coordinator and other interested parties to coordinate efforts, resolve operational problems and improve quality of care.
Bachelors degree in a related field and two years of related experience OR Masters degree in a related field OR one year at the lower level or equivalent position
Licensed Social Worker
Two years experiance providing therapeutic counseling, case management and coordination services, and assessments for court depositions, legal proceedings, and/or eligibilty determinations. One year of experience in human services setting, requiring use of clinical assessments, client interviewing, and/or written reports of assessment findings. Experience in the field of mental retardation and developmental disabilities highly desirable. Computer experience, which iincludes Microsoft Word, Excel, E-mail.

This position is a pay grade 14 on the statewide salary plan.
