The Nurse Practitioner will provide care to seniors in the nursing home including diagnosis, treatments and consultations.  This individual will be responsible and accountable for evaluation, follow-up, and health education regarding residents that are admissions, readmissions, experiencing an acute episode. 

Essential Duties:
  • Initiate, evaluate, review and change orders for selected medications and diagnostic studies/treatments within attending physician supervision and approved protocols.
  •  Perform health histories, physical exams and patient teaching.
  • Order, interpret and follow up on lab tests and results. 
  • Review admissions/readmissions within established timeframes after admission to assure continuity of orders, complete physical assessment and communicate with responsible party as follow-up after admission.
  • Attend care conference, morning meeting and other resident care conferences upon request

Experience Required

The qualified candidate must be have a Bachelor degree in Nursing from accredited college/university; graduate of a certified or accredited Nurse Parctitioner Program. The candidate must have 5-7 years nursing experience with 3 years experience as Nurse Practitioner with the geriatric population, preferably in a long-term care setting.
